PAYMENT INFORMATION

Cost: $95.00 per workshop (breakfast and lunch included)

Three (3) workshops or more $80.00 each
Student*/Senior Citizen rate is $60.00 per workshop.
* (Copy of Student ID MUST accompany registration)

Group Discount (3 or more employees from same agency registered for same workshop) $80.00 each.

Payment may be made by check, money order, Visa or Master Card. DO NOT SEND CASH. Make check payable to Haymarket Center; your check is your receipt. Registrations made using a credit card will be sent confirmations. Parking is free.

CONTINUING EDUCATION CREDITS (CEUS)

Five CEUs will be awarded from the Illinois Alcoholism and Other Drug Abuse Professional Certification Assoc., Inc.(IAODAPCA), CADC, Illinois Department of Professional Regulations for Social Workers, L.P.C. and L.C.P.C. for each workshop
